What is Assessment?
Assessment aims to assist you to chose available care which best suits your needs. It is a consultation between you and care workers.
- Low care assessment is necessary for Hostel placement
- High care assessment is necessary for Nursing Home placement
Assessment is carried out by an Aged Care Assessment Team (ACAT). In this area, Northern Rivers NSW, the team are located at Ballina Hospital and can be contacted by telephoning (02)6620 6222.
- ACAT recommends whether Nursing Home (high care) or Hostel (low care) is the most appropriate care for you.
- They do this by reviewing your physical, mental and social functioning.
- They then advise whether you are capable of living independently or require the support of Hostel (low) or Nursing Home (high) care.
- The ACAT may recommend you do not need Hostel or Nursing Home accommodation.
